Prescription & Nonprescription Medication Requirements
School personal may administer prescription and nonprescription medication with appropriate authorization. Please read the following and use the forms provided below.
- For administration of any nonprescription drug products, school districts must obtain the written instruction and consent from the student's parents or guardians. Parental written requests to administer nonprescription medication in a dosage other than the recommended therapeutic dose must also be accompanied by the written approval of the student's medical provider.
- Prescription drugs may be administered with written instructions of a medical practitioner and the written consent of the student's parents or guardians. Practitioner is defined as a physician, dentist, optometrist, physician assistant, advanced practice nurse practitioner, and podiatrist.
Packaging of Medication:
- Nonprescription medication must come in the manufacturer's packaging with a list of ingredients and recommended therapeutic dose in a legible format.
- Prescription medication must come in t the original pharmacy-labeled package and the label must specify the name of the student, name of the provider, prescribed drug's name, dose, effective date, and directions for administration in a legible format.
